  • (Ron speaks quietly so as not to interrupt the other mechanics hard at work in this auto shop.) Hi! I’m Ron Krier from Ron’s Auto Glass. I’m replacing a windshield today from a 2005 Chevrolet Pickup. This was done before… About a year ago. And on this particular model, it was primed but they did not get all the rust spots, they did not get all the rust spots the knife got. What happens is that the metal rots out. Right away! This glass installation is only a year old, and you can see the rust coming in here, from not being primed all the way up. I put some knife marks in there too, as well, to show you the difference. But I will prime mine, where the previous glass installer didn’t prime theirs. That is the problem with doing auto glass and “cutting corners.” I want to let you know that I don’t “cut any corners.” I prime everything after it has been cut open, and before the glass is re-installed.

    (Ron speaks quietly again.) I just showed you before the scratches that were in this roofline of this 2005 Chevrolet Pickup Truck. I have now primed the area, all the way across the roofline because there were scratches from a previous glass installation. Now, I have primed the complete area that was covered with rust and scratches. It is now safe for the windshield to be installed into this vehicle. I use Sika Urethane and Glue. It works well together and I have zero problem recommending this product to any of my customers, when they need something fixed or done properly. Ron Krier from Ron’s Auto Glass.     What are form stable implants and why are they called Gummy Bear implants?

    Form stable implants are silicone implants with a textured surface and anatomical/teardrop shape that are made with a more cohesive/solid gel than traditional silicone implants. They have recently been FDA approved for use by all plastic surgeons in the US who complete necessary training.

    How is the form stable silicone different from regular silicone?

    The first generation of smooth, round silicone implants from the 1960s contained a thin silicone shell filled with a thin gel similar to honey. Smooth, round implants have been redesigned multiple times, and each time a higher percentage of the silicone has been chemically cross-linked to create a thicker gel. The shell has been made slightly thicker as well to resist rupture.

    The gel in smooth, round implants behaves a little more like a solid, but the gel still “flows” within the implant. When the implant is upright, the gel flows to the bottom, which decreases fullness in the top of the breast and can even lead to visible rippling in very thin patients.

    The silicone in form stable implants has even more cross-linking and behaves more like a solid. They are called Gummy Bear implants because that is what they feel like outside of the body.

    Who makes Gummy Bear Implants?

    There are three companies with form stable implants for use in the US. Sientra was the first to obtain FDA approval in March 2012. They do not manufacture the implants, but they are an importer and distributor for Silimed, which is the most popular implant manufacturer in Brazil. Approval was obtained for both Allergan’s Natrelle 410 device and Mentor’s MemoryShape CPG device in 2013.

    What sizes are offered?

    All implants vary in height, width, and projection. This means that for a given volume, say 300mL, there are many implant shapes to choose from, all of which will give a different appearance to the breast. Some have a more exaggerated teardrop shaped, and some are more round. Measurements of the breast, the amount and firmness of the existing breast tissue and skin, as well as patient opinion about size all play a role.

    Sientra currently offers five shapes, Natrelle offers twelve shapes, and Mentor offers six shapes. There are more shapes available in other countries, and the manufacturers all have ongoing studies so that the other shapes (larger sizes with more projection) can hopefully be released for general use in the US soon.

    What are the advantages of form stable implants?

    One of the advantages of form stable implants is that they match the shape of the natural breast, giving more fullness to the bottom of the breast. The firmer gel leads to more stability in the vertical position with less wrinkling and more stable fullness in the upper part of the breast.

    The purpose of the textured surface is to prevent the implant from rotating, but it also reduces the risk of capsular contracture, which is thickening of the normal scar tissue capsule that forms around the implant. Form stable implants also have a much lower risk of rupture than traditional silicone implants, and if they do rupture the gel is much less likely to migrate out of the capsule into the breast tissue.

    What are the disadvantages of form stable implants?

    One disadvantage is potential rotation of the implant. If the implant moves after placement, the thicker part of the implant may not be at the bottom of the breast where fullness is desired, and this can distort the breast’s shape.

    Also, because the textured surface means the implant is “locked” into place on the chest, some patients complain that the breasts are “unbouncy” and that it is difficult to create cleavage when compared with smooth, round implants.

    Finally, because the gel is thicker, a slightly longer incision is needed to insert them, and some women complain that the breasts feel hard.

    Who is a good candidate?

    The best candidate is a woman over age 22 (the FDA limit for silicone implants in general) with little or no sag to the breast who wants a natural appearance with a moderate increase in size (at least until higher projection styles are approved in the US). The implants can be used in reconstruction cases as well as with patients who require a breast lift, but surgeon experience is critical to ensure a natural looking result without complications like rotation.
