You can see that firstly, she squeezes the sides of the bottle, and then puts her breast into the middle of the breast pump. Squeezing the sides of the bottle is how she gets suction on the pump.
Then you can see that due to the suction, her nipple extends into the pump.
She can leave it like this to catch natural let-down as she breastfeeds her baby from the other side.
Or she can continue to gently squeeze the sides of the bottle to manually express milk.
